To kowtow is to kneel, with your forehead touching the ground, with both hands placed together in a praying position over your head. Then one prays to the Person specified. The Six Kowtows are to be practiced before the Blessed Sacrament, the altar, a crucifix, a Divine Mercy picture, or a picture of the Sacred Heart of Jesus and the Immaculate Heart of Mother Mary. Unite soul, body and heart - in submission, in prostration, in worship - wholeheartedly repenting, praising, honoring, extolling, thanking God with each kowtow.
After each kowtow say "Totus Tuus," which means I totally belong to each Person. If physically you cannot prostrate then see yourself in this position worshipping with your heart, with your soul, offering to God the Six Kowtows spiritually.

And I heard every creature in heaven and on earth and under the earth and in the sea, and all that is in them, saying, “To him who sits on the throne and to the Lamb be blessing and honor and glory and might forever and ever!” (Revelation 5:13)
From the very beginning of this mission, the Lord has been calling all humanity to humble himself, and to honor our Lord.
He called Lucia many years ago to practice this gesture before she knew His bigger plan. He taught her humility and sometimes later He invited her to spread this throughout the world, with the guidance of His Mother, Blessed Virgin Mary.
His mother said to the servants, “Do whatever He tells you.” (St. John 2:5)>
